Why store injected text at all?

Rejecting it would hide the attempt. Stored and quarantined, it is evidence a person can review.

Can the agent see that something was quarantined?

Yes: id, status and reason, never the text.

Is a model involved?

No. Screening, ranking and contradiction flags are deterministic.
