ToolRuleRequest (required)

`effect` required	string	allow \| deny \| require_approval \| modify (AARM MODIFY: the call proceeds with the transform applied; other rules judge the result)
`priority`	integer	lower runs first; first match wins default `100`
`match_path`	string or null	glob, e.g. /tools/payments/*
`match_service`	string or null
`match_agent`	string or null	glob over agent_id
`conditions`	array of `ToolRuleCondition`
`transform`	array of `ToolRuleTransform`	modify rules only: steps applied in order
