POST /fabric/context/context/audit

Audit an agent's live context window for health issues.

Checks: - Redundancy: near-duplicate items via TF cosine similarity - Staleness: items not referenced in N steps - Omission: references not present in context

Returns health score 0-1 and detailed findings.
