GET /fabric/decisions/stream

GET /fabric/decisions/stream
Pushes each new decision for the caller's tenant as it is recorded.
Authenticated with the API key, or with a read-scoped session token in ?session= (a browser EventSource cannot send headers; the token is short-lived and read-only). Only the caller's tenant is ever streamed. Ends after STREAM_MAX_SECONDS; clients reconnect with ?after=<last seq>.
